Social Work's Unique Role in Community Based Palliative Care

A key aspect of the changing landscape for health care delivery is a trend toward shifting care for certain conditions away from the hospital setting and into the community. As communities across the country are preparing for—and responding to—these changes, CBPC has emerged as a model for helping provide care that is aligned with individual goals and wishes, keeps patients as healthy as possible for as long as possible, and helps mitigate the rising costs of care for serious and advanced illness.
CBPC creates a unique opportunity for social workers to lead this change by leveraging our person-in- environment approach. A number of national studies point to the need to integrate social care with medical care for those with serious and advanced illness in hospital-, hospice- and home-based settings. However, little attention is paid to how communities organize themselves to deliver care in the home-based setting, let alone how to develop and sustain payment models that will support the full suite of services identified in CBPC models. This webinar will include examples of how some communities are addressing these challenges with a view toward the crucial role of social work.
Learning Objectives
At the end of the presentation, the participant will be able to:
- Describe differences between Hospice and Palliative Care (PC), and Community Based Palliative Care (CBPC) as a growing field within this practice area.
- Explain emerging models/best practices for CBPC that address micro, meso and macro social work perspective.
- Identify unique roles for Social Workers in helping communities prepare for and deliver CBPC that impact needs of patients, families and caregivers as they navigate serious and advance illness or end of life from a home based setting.

Jackie Ogg, MSW, has spent the last 30 years in the field of social services bridging direct service, philanthropy and community engagement. She has held senior leadership positions at Catholic Charities of the Archdiocese of Washington and the Community Foundation for the National Capital Region. Jackie is currently a consultant whose clients include the University of Maryland School of Social Work, Montgomery County Palliative Care and End of Life Coalition and Voice Your Choice, a local campaign to increase participation in Advance Care Planning for anyone 18 years of age or older. |

GRIEVANCE POLICY: ASOW will comply with all legal and ethical responsibilities to be non-discriminatory in promotional activities, program content and in the treatment of program participants. The monitoring and assessment of compliance with these standards will be responsibility of the Education Director in consultation with the Education Chair and clinical director from committee. We do our best to anticipate problems and, when they do present themselves, will attempt to alleviate complaints as quickly as possible. When a complaint, either verbally or written, is received, the following guidelines are followed with respect to achieving resolution: (1) Complaints relative to a speaker or workshop leader, contents of instructional materials being presented, or an individual educational style being utilized, the individual voicing disapproval is requested to place his/her comments in writing. An AOSW representative will convey these comments to the speaker, assuring the confidentiality of the grieved individual. (2) If the complaint concerns a continuing education activity, its content, level of presentation, or facilities in which the event is being held, the representative in charge will attempt to resolve the matter as expeditiously as possible. If the offered resolution (resolutions offered may include, but not be limited to partial/full refund of registration fees paid; credit toward future event) not satisfactory to the individual filing the complaint, then further action may be taken as follows. (3) The individual is requested to place his/her complaint in writing to the attention of the Education Director, The Association of Oncology Social Work, 111 W. Jackson Blvd. Suite 1412, Chicago, IL 60604. The complaint is then reviewed by the Education Director in consultation with the Clinical Director and Education Advisor. A written response will be issued within 30-days of receipt of the written complaint.
